Make a referral

Refer a worker to Redux Rehab

Insurers and case managers refer workers to us. Employers, treating practitioners and workers can ask for workplace rehabilitation here too: we obtain the worker's signed consent and seek the insurer's approval before anything starts. Workers referring themselves sign their consent right here.

Referral form

Every field helps us move faster, but send what you have: we will chase the rest.

Referring on behalf of a worker? Include their mobile and email if you have them: we send the worker our consent form to sign, and the referral goes to the insurer for approval once it is signed.

SIRA approved workplace rehabilitation provider, Same Employer stream. CTP referrals require no separate approval.

Are you the injured worker? Choose Worker (self referral) under Your role and this form becomes your consent form, the same one as reduxrehab.com.au/consent.

Prefer not to use the form? Email [email protected] or call (02) 8403 2320. Please do not include sensitive health detail beyond what the referral needs.

What happens next

  • Within one business day: we acknowledge the referral and ask for anything missing.
  • Within two business days: we confirm suitability either way and send the worker our consent form. Once it is signed, the referral goes to the insurer for approval, because under both schemes the insurer approves the referral and the program.
  • After approval: we contact the worker, the employer and the treating doctor and schedule the initial assessment. The initial report, with findings, barriers and a recommended plan, reaches the insurer within ten business days of first worker contact.

Has the worker already agreed?

If the worker has verbally agreed to Redux Rehab as their rehabilitation provider, send them our consent form to sign on their phone: reduxrehab.com.au/consent. Their referral lands with us the moment they sign.
